This progress defines how she feels towards the player, by the particular ways you interact with her, along with how often you do these interactions. Your affection can change positively or negatively, depending on the type of action you relay towards Monika.
To my knowledge, you unlock the outfits during certain events and then at 1000 affection, Monika will change clothes for you whenever you want. At least, to my knowledge.
